Hidan Uses Voodoo Jutsu (Naruto)
One of the most odious of the Akatsuki is Hidan. His membership in the Jashin cult not only made him truly immortal but also provided him with certain special Jutsu to employ in combat. Hidan enjoys hurting himself in order to inflict greater pain on his adversaries.
Scaring them and playing with their fears by predicting their imminent destruction. This is how he killed Asuma Sarutobi, and he almost killed a few more people before Shikamaru Nara stopped him.
Demon Lord Clayman Is A Dark Puppetmaster (That Time I Got Reincarnated As A Slime)
The demon lord Clayman looks good, but that’s about all you can say about him. As an Isekai villain, Clayman is almost comically nasty and vicious. No amount of repentance or peace offerings would have won him over Rimuru Tempest.
Clayman is the dictatorial type, with an attitude that treats his underlings as puppets and weapons to be used against his opponents. That’s already quite nasty, but he might take it a step further and have his subordinates deliberately mutate or kill themselves to gain even more strength. While he had Milim under his control, he whacked her around.
Szayelapporo Grantz Treats Torture Like A Game (Bleach)
It’s possible that Aizen’s 8th Espada, Szayelaporro Grantz, isn’t his most potent minion. However, he stands out as the cruelest of the bunch. He is a ruthless scientist and weapons designer who sees everyone as a potential subject for his research and development.
When battle begins, Szayelaporro will transform into his freed form and use its powers to ensnare his enemies, then use a voodoo doll to slowly poison and murder them. He was certain to drive Renji and Uryu to the point of death with their torture. That’s sick, and to think he found amusement in it.
Cioccolata Drove His Patients Over The Edge (JoJo’s Bizarre Adventure)
When Cioccolata was a surgeon many years ago, he didn’t give a damn about the Hippocratic Oath. He became into a sick sadist who would routinely abuse his elderly patients verbally and mentally to the point that they would be on the verge of breaking down. Causing his unfortunate victims to despair to the point of suicide at his hands.
That is frightening on its own, and Cioccolata wasn’t even through yet. He eventually started using Stands, and while fighting Giorno’s organization, he caused widespread illness and death with his lethal Stand, Green Day. If he hadn’t been stopped, he might have gone on to kill even more innocent people.

Donquixote Doflamingo Is A Sadistic Tyrant (One Piece)
“Saw-tooth” Arlong, “Gecko” Moria, and “Captain” Blackbeard are just a few of the numerous gruesome and depraved villains who have appeared in One Piece. The bright blond Doflamingo stands out as the only one who can truly be called nasty and terrible. In the New World part of the story, his atrocities were revealed in their entirety.
Doflamingo’s initial wicked act was to mock the slaughter that occurred during the Marineford conflict. The plot then revealed Doflamingo to be the despotic ruler of Dressrosa. He finds gore and blood to be entertaining, but Luffy disagrees.
